The Support at Home Program represents a significant shift in aged care delivery, consolidating the CHSP and HCP systems into a single, more flexible model. As this reform rolls out nationally, service providers must be ready to offer holistic, person-centred supports, including mental health care.
Through the Woods Counselling is prepared to support older Australians within the new Support at Home framework. Our mental health and expressive therapy services are designed to support independence, community connection, and emotional wellbeing—all key pillars of the new program.
We offer in-home, in-community, and telehealth options, recognising the need for flexible and responsive care. Our services address key risk factors for poor mental health in older adults: social isolation, grief, chronic pain, and identity loss. These services can be provided individually or as group programs.
Mental health and wellbeing supports under the new model are essential for quality of life outcomes and are consistent with the Aged Care Quality Standards. Our programs can be built into a provider’s service delivery plan, offering weekly or fortnightly emotional support sessions.
Sample activities include:
- Life review and reflection therapy
- Reminiscence-based art therapy
- Guided relaxation and mindfulness
- Family dynamic counselling and legacy planning
We partner with care providers to ensure coordination of services, report regularly on progress and outcomes, and participate in care team reviews. Our documentation and feedback loops assist with compliance and quality reporting.
Example: Alan, 76, living alone, began sessions after reporting anxiety and sleep issues. Through bi-weekly therapy and guided art sessions, he developed a night routine, processed grief over a sibling’s passing, and began weekly community outings. His wellbeing scores improved within 3 months.
